Privacy and Disclosure
Mango Lemon will collect relevant personal and sensitive information enabling the delivery of services offered. This includes collecting your personal data such as contact information, payment details, and relevant health information.
Information is held securely through third party databases known as Gymmate and Ezidebit to manage the security of your private and sensitive information.
Informed Consent and Assumption of Risk:
I agree to advise the gym in writing, of any medical conditions that may impact my ability to safely use the gym and it's equipment.
I consent to receive medical treatment which may be deemed advisable in the event of injury, accident and/or illness whilst on the premises. It is suggested that all persons seek medical advice and obtain a medical clearance prior to engaging in physical exercise.
I am aware that the use of the gym and its facilities involves activity that can be physically demanding and that exercise and the equipment is to be used within my capability.
I agree that I am in a good state of health and I am medically fit to use the gym facilities and there is no medical reason to prevent me from proceeding with the use of the gym facilities without endangering my health.
I acknowledge that the club cannot warrant the safety and suitability of the Gym equipment.
I hereby assume all risks associated with the use of premises and facilities.
Media Consent:
I hereby give permission for images of myself, and/or my child, captured during regular and special activities, through video, camera and digital camera, to be used for the purposes of CrossFit promotional material and social media, and waive any rights of compensation or ownership thereto. Last names of minors will not be given or posted on the internet or website.
If you do not wish to be filmed or photographed, it is your responsibility to notify the Coach at the beginning of the class.
Membership Terms, Cancellations and Refunds:
I will provide a minimum of 2 weeks notice when cancelling my membership, unless my membership is a fixed period membership or a 10 pack membership, whereby no notice is required and the membership will end at the scheduled end date.
Memberships can be placed on hold for a maximum period of 4 weeks per Calendar year. Thereafter, each week of membership placed on hold will incur a $10 hold fee per week, to cover administrative costs.
Hold periods cannot be applied on any fixed time special offers.
Refunds will only be provided where an error has occurred. This includes where you have been charged membership fees during a valid hold period.
If your membership is cancelled by the gym due to a breach in these terms and conditions, no refunds will be applicable to any fees paid in advance.
No refunds will be payable if you cancel your membership and have paid any membership fees in advance.
